Hong Kong Youth Squad Team 1 and 2 Withdrawal Form Sample and Withdrawal Procedure

1. A written request for withdrawal MUST be submitted.

2. The written request for withdrawal MUST be signed by the parent/guardian.

3. All written request for withdrawals must be prepared in accordance with withdrawal form format on the website of HKAA and submitted by e-mail, fax, personally or post to the Hong Kong Archery Association for transfer to Youth Development Director, Ms. Po Yin Chun.

4. Official confirmation and approval of the withdrawal via email will be replied by secretariat.
